Change or Correct Passports

Note:

  • Passports can no longer be amended; therefore you will need to apply for a new passport.
  • You must provide us with the original documents and their photocopies (single-sided). Original documents will be returned to you after reviewing your application.
  • Processing time is about 6-8 weeks.

Change/correction is within one year after issuance

If the change/correction is within one year after issuance, you may apply by mail (for both adults and minors age 16 and under).

Please send the following items to the Consular office in your district.

  • Valid passport (original + 1 photocopy)
  • A completed form DS-5504
    Complete online, print out single-sided, and sign. If applying for children under 16, a parent should sign the application form on behalf of the child. (How to complete the online application form)
    *NOTE: You must provide a Social Security number, if you have been issued one. If you do not have a Social Security number, please click here.
  • For corrections: Evidence of correct biographic information (original + 1 photocopy)*
    e.g. Original birth certificate, previous passport
  • For name changes: Legal documentation of name change (original + 1 photocopy)*
    e.g. Original marriage certificate, divorce decree, court order
    *All foreign documents must be accompanied by an English translation (formal or informal). Click here for translation templates for Japanese documents.
  • One passport photo
    ONLY WHITE backgrounds will be accepted (blue backgrounds will be rejected) / 2″×2″ in size / NO eyeglasses / NO uniforms
  • One self-addressed “Japan Post LetterPack envelope”
    (please fold it in half if necessary)
    LetterPack LIGHT: 370 yen, traceable, signature not required
    LetterPack PLUS : 520 yen, traceable, signature required
    For persons using an APO/FPO address, bring one self-addressed envelope that will be mailed via Military Postal Service (MPS). Please note that MPS envelopes are not trackable.  The Embassy cannot mail MPS envelopes using tracking numbers or by registered/certified mail.
  • No Fee (if the correction is within 1 year after issuance)

Change/correction is more than one year after issuance

If the change is more than one year after issuance, please follow the procedures for passport renewal and include:

  • For corrections: Evidence of correct biographic information (original + 1 photocopy)*
    e.g. Original birth certificate, previous passport
  • For name changes: Legal documentation of name change (original + 1 photocopy)*
    e.g. Original marriage certificate, divorce decree, court order
