Replacing a Lost/Stolen Passport

APPOINTMENTS ARE REQUIRED FOR THIS SERVICE

Unless you qualify to mail in your passport application, appointments are required.

Note:

  • If your passport is lost or stolen, you are advised to report the loss/theft to the local Japanese Police Station and obtain a police report.
  • Original documents will be returned to you after review of your application.
  • Processing time is about 6-8 weeks.

Adults (age 18 and older)

Applicants must apply in person. Appointments are required.

  • Proof of U.S. Citizenship
  • One valid proof of ID
  • One passport photo
    ONLY WHITE backgrounds will be accepted (blue backgrounds will be rejected) / 2″×2″ in size / NO eyeglasses / NO uniforms
  • A completed form DS-11
    Complete online and print out single-sided. (How to complete the online application form)
    *NOTE: You must provide a Social Security number, if you have been issued one. If you do not have a Social Security number, please click here.
  • A completed form DS-64
  • One self-addressed “Japan Post LetterPack envelope
    LetterPack LIGHT: 370 yen, traceable, signature not required
    LetterPack PLUS : 520 yen, traceable, signature required
    For persons using an APO/FPO address, bring one self-addressed envelope that will be mailed via Military Postal Service (MPS). Please note that MPS envelopes are not trackable.  The Embassy cannot mail MPS envelopes using tracking numbers or by registered/certified mail.
  • Passport Book $165
    Pay the passport application fee via the  U.S. government’s secure payment sitePlease schedule your appointment at least 5 working days after payment submission.  This provides time for your payment to be processed prior to your passport interview.  If your payment hasn’t cleared by the time of your appointment, we will be unable to process your application, and you will need to return to the Embassy or Consulate in your district at a later date once payment has cleared.  After successfully paying the fee, you must print out the electronic payment confirmation you receive via email and submit it on the day of your appointment.  Paying online allows you to avoid long wait times during your appointment at the U.S. Embassy.  If you cannot provide all the necessary documents, please consult with our staff on the day of your appointment.  Please click HERE for more information on fees and for acceptable forms of payment. If you have urgent travel plans within 2 weeks, please DO NOT submit your passport payment online.

Children under 16

Applicants (children) must appear in person accompanied by both parents. Appointments are required.

  • Proof of U.S. Citizenship

    –U.S. birth certificate
    –Consular Report of Birth Abroad
    –Foreign birth certificate (e.g. Family Register “Koseki Tohon”)
    –Adoption decree
    –Divorce/Custody decree
    Note: all documents must be original – no photocopies. All foreign documents must be accompanied by an English translation.
  • Parents’ valid IDs (original + 1 photocopy)

Applying as a 16 or 17 year old

  • Parental Consent
    You must show at least one legal parent or legal guardian is aware that you are applying for a passportHow to Show Parental Awareness
    – A legal parent or legal guardian appearing with you in person when you apply for your passport. The employee accepting the application will ask your legal parent or legal guardian to sign Form DS-11. Bring a photocopy of that parent or guardian’s ID.
    – Submit a signed note from your legal parent or legal guardian with a photocopy of their ID.
    – We may ask you to submit a notarized statement from your legal parent or legal guardian (e.g., on Form DS-3053) which states he or she supports you receiving a passport. The statement must be accompanied by a photocopy of that parent or guardian’s ID.
