Change or Correct Passports

The U.S. Embassy in Tokyo has resumed some routine services. To schedule an appointment, please click here. If the appointments are fully booked, please check our website at the beginning of each month for the following month’s appointments. To apply for a social security card, please DO NOT make an appointment here. Please contact the Federal Benefits Unit at FBU.Tokyo@ssa.gov

Online Appointments are Necessary:

Tokyo    • Sapporo    • Osaka/Kobe    • Fukuoka    • Naha

  • If you have changed your name or need to correct data on your passport, you will need to have this change reflected in your passport. Please note that passports can no longer be amended; therefore you will need to apply for a new passport.
  • Original documents will be returned to you after review of your application.

If the change/correction is within one year after issuance, you may apply by mail (for both adults and minors age 16 and under). 

Please send the following items to the Consular office in your district.

  • Valid passport
  • Form DS-5504
    Complete online, print out single-sided, and sign. (How to complete the online application form)
    *NOTE: You must provide a Social Security number, if you have been issued one. If you do not have a Social Security number, please click here.
  • For corrections: Evidence of correct biographic information (original + 1 photocopy)*
    e.g. Original birth certificate, previous passport
  • For name changes: Legal documentation of name change (original + 1 photocopy)*
    e.g. Original marriage certificate, divorce decree, court order
    *All foreign documents must be accompanied by an English translation (formal or informal). Click here for translation templates for Japanese documents.
    *Original documents will be returned to you after review of your application.
  • One passport photo 
    White background / 2″×2″ in size/ no eyeglasses /no uniforms
  • One “Japan Post LetterPack envelope” (self-addressed) 
    (please fold it in half if necessary)
    LetterPack LIGHT: 370 yen, traceable, signature not required
    LetterPack PLUS : 520 yen, traceable, signature required
    For persons using an APO/FPO address, bring one self-addressed envelope that will be mailed via Military Postal Service (MPS).

If applying for children under 16, a parent should sign the application form on behalf of the child.

If the change is more than one year after issuance, please follow the procedures for passport renewal (Age 16 and up / Children under 16 ) and include:

  • For corrections: Evidence of correct biographic information (original + 1 photocopy)*

  • For name changes: Legal documentation of name change (original + 1 photocopy)*
