Replacing a Lost/Stolen Passport

NOTICE: During Japan’s State of Emergency, the U.S. Embassy and Consulates strongly encourage our customers to make appointments at the location nearest them. Applicants should avoid traveling throughout Japan solely for the purpose of obtaining a Consular Report of Birth Abroad, passport, or notarial service appointment. If the Consulate nearest you does not have appointments available online and you have an urgent need for service, please contact us at the email addresses listed below.

Appointments at the U.S. Embassy in Tokyo:

The U.S. Embassy in Tokyo has resumed some routine services.  To schedule an appointment, please click here. If the appointments are fully booked, please check our website at the beginning of each month for the following month’s appointments. If you require an emergency appointment, please email TokyoPPT@state.gov. For applicants who used the electronic application for the Consular Report of Birth, please allow 5 working days between payment submission and interview appointment time. Please note that mail-in renewal is available for most adult passports (see more information).

To apply for a social security card, please DO NOT make an appointment here. Please make an appointment from this link. It is no longer possible to apply for a social security card at the same time as a Consular Report of Birth Abroad.

Appointments in Tokyo, Sapporo, Osaka/Kobe, Fukuoka, and Naha:

Online appointments are now available for Tokyo, SapporoOsaka/KobeFukuoka, and Naha. In order to avoid unnecessary travel, please make an appointment at the nearest location.

  • If your passport is lost or stolen, you are advised to report the loss/theft to the local Japanese Police Station and obtain a police report.
  • Original documents will be returned to you after review of your application.

All applicants must apply in person.

  • Proof of U.S. Citizenship (if circumstances allow)
  • One valid proof of ID (if circumstances allow)
  • One passport photo 
    White background /2″×2″ in size / no eyeglasses / no uniforms
  • Form DS-11 
    Complete online and print out single-sided. (How to complete the online application form)
    *NOTE: You must provide a Social Security number, if you have been issued one. If you do not have a Social Security number, please click here.
  • Form DS-64
  • One “Japan Post LetterPack envelope” (self-addressed) 
    LetterPack LIGHT: 370 yen, traceable, signature not required
    LetterPack PLUS : 520 yen, traceable, signature required
    For persons using an APO/FPO address, bring one self-addressed envelope that will be mailed via Military Postal Service (MPS).
  • Passport Fee

All applicants (children) must appear in person accompanied by both parents.

    White background, 2″×2″ in size, no eyeglasses, no uniforms
  • Form DS-11

  • Form DS-64
  • Proof of parent-child relationship (if circumstances allow)
    –U.S. birth certificate
    –Consular Report of Birth Abroad
    –Foreign birth certificate (e.g. Family Register “Koseki Tohon”)
    –Adoption decree
    –Divorce/Custody decree
    Note: all documents must be original – no photocopies. All foreign documents must be accompanied by an English translation.
  • Parents’ valid IDs (original + 1 photocopy)
  • One “Japan Post LetterPack envelope” (self-addressed)
